Position Overview & Specifications
The post holder will assess, plan, implement and evaluate client care supported through a robust preceptorship or personal and professional development programme linked to the Trust’s implementation of the Knowledge and Skills.
The post holder will also co-ordinate the individual’s assessment and subsequent care plan as agreed with the multi-disciplinary team.
The post holder will adhere to Trust policies, Data Protection and Health and Safety.
- To practice core values of recovery by focusing on improvement in functions rather than symptoms and assisting in developing social and occupational roles for service users.
- Establish effective working relationships with service users, carers and families and encourage their participation in the recovery process
- To assess, plan, implement and evaluate client care that is based on best practice and delivers high standard of nursing care.
- To co-ordinate the individual’s assessment and subsequent care plan as agreed with the multi-disciplinary team.
- To effectively communicate with clients, family and colleagues
- To work in partnershipwith service users and carers, rehabilitation service managers and third sector who provide supported accommodation and vocational rehabilitation.
- Ensure that statutory and legal requirements of care are incorporated safely within each patient’s programme of care.
- To meet the physical, psychological, social and cultural needs of the individual and their family, using research based knowledge and skills to maintain the individual’s identity.
- Observe the requirements of the Mental Health Act 1983 ensuring the patient is fully informed of their legal position and their rights under the Act.
